OHSU brings you safe, excellent care — in person and in virtual visits. Call your clinic or see MyChart for details. This application process allows you to compile your application materials once and apply to multiple programs with one, centralized application. The application fee is based on the number of programs to which an applicant applies. An application must be verified by our deadline in order to be considered. Please keep this in mind when planning your application to the program. We recommend submitting your application weeks ahead of the deadline to allow extra time just in case! It is up to the applicant to ensure their application meets the program's deadlines. There is no supplemental application.
Click here to start your application. It's important you apply as early as possible since completing your application can be a lengthy process, and since application processing times vary throughout the cycle. This timeline may need to be adjusted based on your programs' deadlines and requirements. Submit and complete your application. See Sending Official Transcripts for more guidelines on each type of transcript. The Transcript Request Form is found in the Colleges Attended section, after you submit a college or university entry. You must report all colleges and universities attended and submit official transcripts, even if your transfer credits are listed on your primary transcript. See Sending Official Transcripts for more guidelines.
